Brookhaven Hospital Patient Accounts & Records

The Brookhaven Patient Portal gives patients and approved caregivers secure access to appointment details, admission records, care instructions, billing notices, and messages from Brookhaven Hospital staff.

Portal access is available to current and former Brookhaven patients, including patients transferred from Alchemilla Hospital, Cedar Grove Sanitarium, or other Silent Hill medical facilities with compatible records.

Note: Some Brookhaven portal services may be delayed during lake fog advisories, townwide siren events, records-room outages, or other unusual service interruptions. If the portal appears distorted, incomplete, or unexpectedly redirects, wait a few minutes before trying again.

Portal service coverage

The Brookhaven Patient Portal may show different services depending on your account type, care history, and record-release status.

Before you begin

To create your Brookhaven Patient Portal account, you’ll need a few pieces of information from your hospital records.

  • Your Brookhaven Patient ID or temporary intake number
  • Your full legal name as listed on your admission paperwork
  • Your date of birth
  • A valid email address or mobile phone number
  • Your most recent discharge, transfer, or appointment paperwork
Important: If you were admitted under emergency observation, psychiatric hold, or unidentified-patient intake, your Patient ID may not match the number printed on older wristbands. Use the most recent number provided by the Brookhaven Records Office.
Tip: If you have been transferred from Alchemilla Hospital, verify that your discharge papers specify Brookhaven as your current care facility. Accounts are tied to the hospital you’re treated at and cannot be shared between institutions.

Create your account

Once you have your patient information ready, follow the steps below.

  1. Go to the portal website. Open a modern web browser and navigate to portal.brookhaven.sh.
  2. Select “Create Account.” On the portal home page, click the Create Account button.
  3. Enter your personal details. Provide your full legal name, date of birth, and contact information.
  4. Verify your facility. Select Brookhaven Hospital as your current or most recent care facility.
  5. Create a username. Usernames must be 4–20 characters and may include letters, numbers, and underscores.
  6. Set a secure password. Choose a password with at least 8 characters, including uppercase letters, lowercase letters, numbers, and at least one special character.
  7. Accept the terms of service. Review the Brookhaven Patient Portal Terms and Conditions.
  8. Submit your registration. Select Create Account. You will receive a verification email or text message with a one-time code.
Tip: Do not use your room number, patient wing, case number, or the name of a treating physician as your password. For example, passwords such as S3Patient, DrKaufmann, or Lakeview312 are not considered secure.
Warning: Creating your account during an active environmental service interruption, records-room outage, or townwide siren event may result in incomplete registration data or lost credentials. Wait until service conditions stabilize before proceeding.

Verify your identity

After submitting your registration, Brookhaven will send you a one-time verification code. You must enter this code before your portal account can be activated.

Email verification

Check your inbox for a message from no-reply@portal.brookhaven.sh. Open the message and enter the verification code on the portal page.

Text verification

If you selected text verification, check your mobile phone for a six-digit code. Enter the code before it expires.

Security reminder: Brookhaven staff will never ask for your password or verification code by phone, email, text, radio transmission, handwritten note, or intercom announcement.

After your account is created

Once your account is active, you can use the Brookhaven Patient Portal to:

  • View upcoming appointments and follow-up visits
  • Review discharge instructions
  • Request copies of medical records
  • Update your contact information
  • Send secure messages to Patient Accounts & Records
  • View transfer notes from Alchemilla Hospital, when available
  • Check billing notices and payment status

Some psychiatric records, restricted treatment notes, and sealed historical case files may not appear in the portal. To request access to restricted records, contact the Brookhaven Records Office directly.

FAQ

I never received my verification code
  • Check your spam or junk folder.
  • Confirm that your phone number or email address was entered correctly.
  • Wait 10 minutes before requesting a new code.
  • If fog advisory conditions are active, try again once service stabilizes.
My Patient ID is not working
  • Use the Patient ID from your most recent Brookhaven paperwork.
  • Do not include spaces or dashes unless they appear in the portal example.
  • If you were transferred from Alchemilla, allow up to 24 hours for records to sync.
  • Contact Patient Accounts & Records if your file was created during emergency intake.
The portal page looks wrong or keeps refreshing
  • Refresh your browser.
  • Clear your browser cache.
  • Try a different browser or device.
  • Wait and try again if the town siren has recently sounded.
  • Avoid using public terminals located in closed wards, basements, or abandoned wings.
I created an account, but I can’t see my records

Some records may take additional time to appear. Newly created accounts may not display historical records, transfer notes, or treatment summaries immediately.

If your account still appears empty after 24 hours, contact Brookhaven Patient Support.
