Link Brookhaven and Alchemilla records under one profile

If you have received care at both Brookhaven Hospital and Alchemilla Hospital, you may be able to link your records under one Silent Hill Healthcare profile. Linking records lets you view eligible appointments, discharge instructions, billing notices, transfer notes, and secure messages from both facilities in one patient portal account.

Record linking is most commonly used for patients who were transferred between facilities, received emergency care at Alchemilla before follow-up treatment at Brookhaven, or have separate historical records under both hospital systems.

Note: Record linking may be delayed during lake fog advisories, townwide siren events, records-room outages, basement archive reviews, or unusual service conditions. If your linked profile appears incomplete, wait a few minutes before refreshing or submitting a duplicate request.

Record linking coverage

Not every record can be linked automatically. The table below explains which records are usually eligible.

Standard records Appointment history, discharge instructions, billing notices, contact information, and routine transfer notes can usually be linked automatically.
Brookhaven records Active mental-health program records, follow-up visits, and patient account details may be linked after identity verification.
Alchemilla records Emergency department visits, inpatient admissions, surgical notes, and transfer summaries may be linked after facility review.
Restricted records Sealed psychiatric notes, basement archive files, historical case materials, and special-case treatment records may require manual review or may not appear in the portal.

Before you begin

Before requesting record linking, make sure you have:

  • Your Silent Hill Healthcare portal username and password
  • Your Brookhaven Patient ID, if available
  • Your Alchemilla Patient ID, if available
  • Your full legal name as listed on both hospital records
  • Your date of birth
  • Your most recent discharge, transfer, billing, or appointment paperwork from each facility
Important: If your name, date of birth, or Patient ID differs between Brookhaven and Alchemilla records, your request may require manual review. Do not create a second portal account unless Patient Accounts & Records specifically instructs you to do so.
Tip: If you were recently transferred between facilities, wait 24 hours before requesting record linking. Some transfer notes may not appear until both Records Offices finish syncing your file.

What happens after you submit a request

After your linking request is submitted, Patient Accounts & Records will review the request and check that the records belong to the same patient.

Most standard requests are completed within 24–48 hours. Requests involving transfer discrepancies, older paper records, restricted treatment notes, or basement archive files may take longer.

  • You may receive a message if additional verification is needed.
  • Some records may appear before others.
  • Billing details may sync separately from clinical records.
  • Restricted notes may remain hidden even after records are linked.
Tip: If only one facility appears after linking, sign out and sign back in before submitting a new request. Recently linked records may take a few minutes to appear in the profile switcher.

FAQ

My Patient ID is not recognized
  • Use the Patient ID from your most recent paperwork for that facility.
  • Do not include spaces or dashes unless they appear in the portal example.
  • If you were recently transferred, wait 24 hours before trying again.
  • Contact Patient Accounts & Records if your file was created during emergency intake.
My records show under the wrong facility
  • Check whether the record is a transfer note from another facility.
  • Confirm that you selected the correct facility in the profile switcher.
  • Wait 24–48 hours for billing and clinical records to sync.
  • Contact Patient Accounts & Records if the record appears attached to the wrong patient.
I cannot see restricted notes after linking records

Some notes remain restricted even after records are linked. This may include sealed psychiatric notes, historical case files, basement archive records, legal holds, or records requiring clinician review.

To request access to restricted notes, contact the Records Office for the facility where the care was provided.

My linking request is still pending
  • Allow 24–48 hours for standard requests.
  • Check your secure messages for requests for additional verification.
  • Requests involving older paper records or restricted files may take longer.
  • If a fog advisory or townwide siren event occurred recently, allow extra processing time.
The Linked Records page looks wrong or keeps refreshing
  • Refresh your browser.
  • Clear your browser cache.
  • Try a different browser or device.
  • Wait and try again if the town siren has recently sounded.
  • Avoid using public terminals located in closed wards, basements, or restricted wings.
