Download an itemized statement
An itemized statement provides a detailed list of charges connected to a visit, account, procedure, emergency department encounter, inpatient stay, or other Silent Hill Healthcare service. You may need an itemized statement for insurance review, reimbursement, flexible spending account documentation, legal records, personal records, or billing questions.
Itemized statements are usually available in the patient portal after charges have been posted and the billing account has been reviewed. Recent visits may not show an itemized statement right away.
Before you begin
Before downloading an itemized statement, make sure you have:
- Access to your Silent Hill Healthcare patient portal
- The visit date or statement date
- The facility name, such as Brookhaven Hospital or Alchemilla Hospital
- Your account number or guarantor number, if available
- A device that can open or download PDF files
Download an itemized statement
To download an itemized statement from the portal:
- Sign in to your Silent Hill Healthcare patient portal:
- Go to Billing, Statements, or Patient Financial Services.
- Select the statement, visit, or billing account you want to review.
- Choose View details, Statement details, or Itemized statement.
- Select Download PDF, Print statement, or Export itemized statement.
- Save the file to your device or print a copy for your records.
What an itemized statement includes
An itemized statement may include:
- Patient name
- Statement date
- Visit date or service date
- Facility or department name
- Account number or guarantor number
- Service descriptions
- Procedure, supply, room, medication, imaging, or lab charges
- Insurance payments or adjustments, if available
- Patient payments or remaining balance
When you may need one
You may need an itemized statement when:
- Your insurance company requests charge details
- You are submitting a reimbursement claim
- You need documentation for an FSA, HSA, or benefits program
- You are reviewing a balance or charge
- You need billing records for personal files
- You are comparing a statement to an explanation of benefits
- An attorney, case worker, or authorized representative requests billing documentation
FAQ
I do not see an itemized statement
The billing account may not be finalized yet. Recent visits, emergency care, inpatient stays, surgical services, imaging, labs, or insurance reprocessing may take longer before an itemized statement is available.
The PDF will not download
- Disable pop-up blockers for the portal.
- Try a different browser or device.
- Check your downloads folder.
- Use Print statement and select Save as PDF, if available.
The charges do not match my insurance explanation of benefits
Insurance explanations of benefits and itemized statements may update on different timelines. Claims, adjustments, payments, denials, and corrected balances may still be processing. Contact Patient Financial Services if the difference remains after the claim review period.
I need an older itemized statement
Older statements may not be available for direct download. Contact Patient Financial Services if you need an archived Brookhaven, Alchemilla, or municipal billing statement.
The billing page looks wrong or keeps refreshing
- Refresh your browser.
- Clear your browser cache.
- Try a different browser or device.
- Wait and try again if the town siren has recently sounded.
- Avoid using public terminals located in restricted billing offices, closed wards, or archive areas.
