My Alchemilla emergency visit is missing from my portal

An Alchemilla Hospital emergency visit may not appear in your patient portal immediately after care. Emergency department records often include triage notes, provider documentation, lab results, imaging reports, medication records, discharge instructions, transfer notes, and billing records that may finish processing on different timelines.

A missing Alchemilla emergency visit does not always mean the visit was lost. It may be delayed, connected to a Brookhaven transfer, attached to a duplicate profile, waiting for provider sign-off, pending lab or imaging review, or still moving through emergency department discharge processing.

Note: Alchemilla emergency records may take longer to appear after observation stays, inpatient admission, Brookhaven transfer, imaging interpretation, lab review, corrected registration, lake fog advisories, or townwide siren events.

Common reasons an Alchemilla emergency visit is missing

Emergency visits may be delayed or missing from the portal for several reasons. Some delays are normal, while others may require account or records review.

Provider sign-off Emergency provider notes, discharge instructions, or procedure documentation may still need to be finalized.
Lab or imaging review Lab results, X-rays, CT scans, or diagnostic reports may need review before the full visit appears.
Brookhaven transfer If you were transferred, admitted, or evaluated by Brookhaven after emergency care, records may appear under both facilities.
Registration correction Emergency registration may need correction if your name, date of birth, address, guarantor, or insurance information did not match your portal profile.
Account mismatch The visit may be attached to a duplicate patient account, former name, dependent account, or unlinked Alchemilla portal profile.
Tip: If you received emergency care and were later admitted or transferred, check both Alchemilla and Brookhaven sections before submitting a missing visit report.

What to check first

Before reporting a missing Alchemilla emergency visit, review your portal for related records.

  • Check Visits, Medical Records, Documents, and Discharge Documents.
  • Search by emergency visit date, discharge date, provider name, or department.
  • Check whether the visit appears under Brookhaven Hospital due to transfer or consultation.
  • Review lab results, imaging reports, and discharge instructions separately.
  • Check whether you are signed in under the correct patient profile.
  • Review dependent or authorized-access profiles if the emergency visit belongs to a minor or dependent patient.
  • Check secure messages for requests for identity verification, insurance details, or release authorization.
  • Allow extra time if the visit involved observation, admission, imaging, outside consultation, or Brookhaven transfer.
Important: If you see an emergency visit that belongs to someone else, stop viewing it and report the issue immediately. Do not download, print, share, or submit screenshots containing another patient’s information unless Privacy Services asks you to.

Emergency record review and release timing

Alchemilla emergency records may appear in stages. You may see billing information, a visit summary, lab results, or imaging reports before the complete emergency provider note or discharge documentation is available.

Additional review may apply when the emergency visit includes:

  • Emergency department triage documentation
  • Observation status or short-stay monitoring
  • Inpatient admission after emergency care
  • Brookhaven behavioral health consultation or transfer
  • Imaging interpretation or diagnostic report review
  • Lab review, corrected results, or addendum notes
  • Medication administration or pharmacy reconciliation
  • Information from EMS, family members, caregivers, witnesses, or outside providers

Report a missing Alchemilla emergency visit

To report that an Alchemilla emergency visit is missing from your portal:

  1. Sign in to your Silent Hill Healthcare patient portal:
  2. Go to Account Problems and Record Mismatch.
  3. Select Missing Alchemilla emergency visit.
  4. Enter the emergency visit date, discharge date, or estimated date range.
  5. Add the provider, department, service type, or reason for visit, if known.
  6. Explain whether you expected to see an emergency visit summary, discharge instructions, lab results, imaging reports, billing record, or all records.
  7. Note whether you were admitted, observed, transferred to Brookhaven, or referred to another department.
  8. Upload supporting documents only if they do not show another patient’s information.
  9. Select Submit review request.

FAQ

I only see billing information, not emergency records

Billing records may appear before emergency provider notes, discharge instructions, lab results, or imaging reports are finalized. Check the medical records and documents areas after the emergency record review period.

My Brookhaven visit is visible, but the Alchemilla emergency visit is missing

If care began at Alchemilla and continued at Brookhaven, Brookhaven admission or treatment records may appear separately from the emergency department record. Alchemilla records may still be under emergency department review, imaging review, lab review, or registration matching.

My lab or imaging results are missing

Lab and imaging results may release separately from the emergency visit. Some results require provider review, corrected-result processing, radiology interpretation, or additional privacy review before they appear.

I had emergency care years ago

Older Alchemilla emergency visits may be stored in legacy billing or medical record systems. These records may not appear automatically in the portal and may require an archived records request.

The visit history page looks wrong or keeps refreshing
  • Refresh your browser.
  • Clear your browser cache.
  • Try a different browser or device.
  • Wait and try again if the town siren has recently sounded.
  • Avoid using public terminals located in restricted offices, emergency back corridors, or archive areas.
