My portal shows an old address or former name

Your patient portal may show an old address, former name, outdated phone number, previous email address, or older demographic information if your profile has not fully updated across all Silent Hill Healthcare systems. This can affect Brookhaven Hospital records, Alchemilla Hospital records, billing statements, guarantor accounts, insurance claims, portal recovery, authorized access, and older archived records.

In many cases, this information can be updated through your portal profile. Some changes, such as legal name updates, billing guarantor updates, dependent account changes, or corrections to older medical records, may require additional review by Patient Accounts & Records, Health Information Management, Patient Financial Services, or Privacy Services.

Note: Name and address updates may appear at different times across portal profiles, medical records, billing records, insurance claims, archived Brookhaven files, Alchemilla emergency records, lake fog advisories, or townwide siren events.

Why old information may appear

Old names, addresses, and contact details may appear when different departments update on different timelines or when older records are linked to your current portal profile.

Profile update delay Your portal profile may update before medical records, billing records, insurance records, or older facility records update.
Legal name review Legal name changes may require identity verification or supporting documentation before appearing on all records.
Billing profile difference Billing statements may use guarantor, insurance subscriber, or responsible-party information that differs from your clinical profile.
Duplicate account Old contact details may appear if some records are attached to a duplicate patient profile or legacy account.
Archived record Older Brookhaven, Alchemilla, or municipal records may display the name or address used at the time of care.
Tip: If the old name or address appears only on an older visit summary or archived document, it may reflect the information on file at the time of treatment rather than a current profile error.

What to check first

Before submitting a mismatch request, check where the old name or address appears.

  • Portal profile or account settings
  • Visit history
  • Visit summaries or discharge documents
  • Brookhaven treatment records
  • Alchemilla emergency or inpatient records
  • Billing statements or itemized statements
  • Insurance information or claim details
  • Authorized users, dependent profiles, or guarantor profiles
  • Older paper records or archived municipal health documents
Important: If you see a name, address, or record that belongs to another patient, stop viewing it and report the issue to Privacy Services. Do not download, print, screenshot, or share another patient’s information unless Privacy Services instructs you to.

Update your name or address

To update your portal name, contact information, or address:

  1. Sign in to your Silent Hill Healthcare patient portal:
  2. Go to Account Settings, Profile, or Patient Accounts & Records.
  3. Select Update name, contact information, or address.
  4. Choose the information you need to update.
  5. Enter the corrected information.
  6. Upload supporting documentation if requested.
  7. Choose whether the update should apply to your portal profile, billing profile, medical record profile, or all eligible records.
  8. Select Submit update.

When additional review is needed

Some updates cannot be applied immediately because they affect medical records, billing records, identity verification, or legal documentation.

Additional review may be required when:

  • You are updating a legal name
  • Your records show both a former name and current legal name
  • Your billing statement uses an old guarantor address
  • Insurance information still shows an old name or subscriber address
  • Brookhaven and Alchemilla records show different demographic information
  • A dependent or minor profile shows the wrong address or guardian name
  • Older paper records show outdated demographic information
  • The old information appears inside a signed medical record or discharge document
  • The old information may be attached to a duplicate patient account
Tip: If the issue appears on billing statements or insurance claims, also update your billing address or insurance information so Patient Financial Services can review the correct account.

FAQ

My portal profile updated, but my bill still shows the old address

Billing profiles may update separately from portal profiles. Submit a billing address update or contact Patient Financial Services if new statements continue using the old address.

My former name appears on older records

Older records may show the name used at the time of care. If the old name appears on new records or causes matching issues, submit a profile update or record correction request.

My insurance still shows the wrong name

Insurance information may depend on the subscriber name and payer records. Update your insurance information and confirm the name listed with your insurance company.

My old address appears on a dependent account

Dependent, minor, guardian, and guarantor profiles may require separate review. Include your relationship to the dependent patient and the affected account area when submitting the update request.

The profile page looks wrong or keeps refreshing
  • Refresh your browser.
  • Clear your browser cache.
  • Try a different browser or device.
  • Wait and try again if the town siren has recently sounded.
  • Avoid using public terminals located in restricted offices, billing desks, closed wards, or archive areas.
