My Brookhaven visit is missing from my portal

A Brookhaven Hospital visit may not appear in your patient portal immediately after care. Some Brookhaven records require provider sign-off, behavioral health review, discharge processing, privacy review, account matching, or archive retrieval before they are visible in your visit history.

A missing Brookhaven visit does not always mean the visit was lost or removed. It may be delayed, connected to another facility encounter, attached to a duplicate profile, pending release review, or stored in an older Brookhaven record system.

  • Allow extra time if the visit involved behavioral health review, discharge planning, or older records.
Important: If you see a Brookhaven visit that belongs to someone else, stop viewing it and report the issue immediately. Do not download, print, share, or submit screenshots containing another patient’s information unless Privacy Services asks you to.

Brookhaven record review and release timing

Brookhaven records may follow different release timelines depending on the visit type. Behavioral health, inpatient, therapy-related, safety planning, medication management, and discharge records may require additional review before they appear in the portal.

  • Psychiatric intake or evaluation
  • Inpatient behavioral health treatment

  • Discharge planning or transfer documentation
  • Information provided by a caregiver, family member, case worker, or witness
  • Older paper files or records from a restricted archive area

Report a missing Brookhaven visit

To report that a Brookhaven visit is missing from your portal:

  1. Sign in to your Silent Hill Healthcare patient portal:
  2. Go to Account Problems and Record Mismatch.
  3. Select Missing Brookhaven visit.
  4. Enter the estimated visit date, discharge date, or date range.
  5. Add the provider, department, ward, service type, or reason for visit, if known.
  6. Explain whether you expected to see a visit summary, discharge document, treatment record, billing record, or all records.
  7. Note whether care began at Alchemilla or involved a transfer.
  8. Upload supporting documents only if they do not show another patient’s information.
  9. Select Submit review request.

FAQ

My Alchemilla visit is visible, but the Brookhaven visit is missing

If care began at Alchemilla and continued at Brookhaven, records may appear on different timelines. Alchemilla emergency documentation may appear first, while Brookhaven admission, treatment, behavioral health, or discharge records remain under review.

My discharge summary is missing

Discharge summaries may require provider sign-off, department review, medication review, or care coordination documentation before portal release. Check again after the review period or submit a missing visit report if the summary does not appear.

I only see billing information, not medical records

Billing records and medical records may appear separately. A charge or statement may be available before visit summaries, notes, discharge documents, or treatment records are released to the portal.

I had care at Brookhaven years ago

Older Brookhaven visits may be stored as paper files or scanned archive records. These may not appear automatically in the portal. You may need to request older paper records from Silent Hill facilities.

The visit history page looks wrong or keeps refreshing
  • Refresh your browser.
  • Clear your browser cache.
  • Try a different browser or device.
  • Wait and try again if the town siren has recently sounded.
  • Avoid using public terminals located in restricted offices, closed wards, or archive areas.
