Complete pre-visit forms and upload documents
Before some Silent Hill Health appointments, you may be asked to complete forms, upload documents, verify insurance, add symptoms, update medications, or provide visit notes through the patient portal. Completing these tasks before your visit can help registration, scheduling, and care teams prepare for your appointment.
Pre-visit tasks may apply to Brookhaven behavioral health visits, Alchemilla outpatient care, emergency follow-up, lab work, imaging, diagnostic testing, procedure follow-up, and specialist referral appointments.
Pre-visit forms you may need to complete
Available forms depend on your facility, appointment type, department, and reason for the visit.

| Insurance verification | Insurance coverage review, subscriber information, billing address, guarantor information, or authorization details. |

Documents you may need to upload
Depending on the visit, you may be asked to upload:
- Photo ID or other accepted identification

- Lab, imaging, diagnostic, or procedure orders

Add symptoms, visit notes, or medication updates
Some appointments allow you to add notes before your visit. These notes help the care team understand what changed since scheduling and what you want to discuss.

- Questions you want to ask during the appointment
- Recent emergency visit, discharge, lab, imaging, or procedure details
- Medication changes, side effects, or refill concerns

- Support person or caregiver notes, if applicable
Complete forms and upload documents through the portal
To complete pre-visit forms or upload documents:
- Sign in to your Silent Hill Health patient portal:
- Go to Appointments, Upcoming visits, or Pre-visit tasks.
- Select the appointment you are preparing for.
- Open Forms, Documents, Check-in, or Upload files.
- Complete any required forms.
- Upload requested documents using clear images or PDF files.
- Add symptoms, visit notes, medication updates, or accessibility notes if available.
- Review your entries and select Submit, Save, or Send to care team.

Document requirements may vary by facility and appointment type.
| Brookhaven Hospital | Brookhaven may request behavioral health intake forms, treatment history, medication lists, safety planning information, discharge documents, authorized-access documents, or records from Alchemilla emergency care. |
| Alchemilla Hospital | Alchemilla may request insurance or ID uploads, emergency discharge instructions, lab orders, imaging orders, procedure instructions, outside records, or referral documents. |
| Shared-care appointments | If your care involves both Brookhaven and Alchemilla, staff may need linked records, discharge summaries, referral notes, or medical release forms before the visit. |
Review and processing times
Some forms and uploads are saved to your chart immediately. Others require review by registration, Scheduling Services, Health Information Management, Patient Financial Services, Privacy Services, or the care team before they apply to your appointment.
Review may be needed when:
- The upload includes insurance, billing, or guarantor information
- The document is a medical release form
- The document includes guardian, dependent, custody, or caregiver information
- The form affects Brookhaven behavioral health privacy review
- The document affects Alchemilla lab, imaging, diagnostic, or procedure scheduling
- Records are attached to the wrong profile or duplicate account
- The document is blurry, incomplete, expired, or missing required pages
- The upload was submitted close to the appointment time
FAQ
I cannot find my pre-visit forms
Check the appointment details, secure messages, pre-visit tasks, and check-in sections of your portal. Some forms appear only after the appointment is confirmed or after staff review.
My upload failed
Try refreshing the page, using a different browser, reducing the file size, or uploading a clearer image or PDF. If the upload still fails, bring the document to your appointment and contact Scheduling Services.
I uploaded the wrong document
Upload the correct document if the portal allows it, then send a message to Scheduling Services explaining which upload should be reviewed. Do not upload another patient’s documents.
I need to add symptoms after submitting my forms
Check whether the portal allows additional visit notes. If not, send a secure message or tell registration and the care team when you arrive.
The forms page looks wrong or keeps refreshing
- Refresh your browser.
- Clear your browser cache.
- Try a different browser or device.
- Check that you are signed in to the correct patient profile.
- Wait and try again if the town siren has recently sounded.
- Avoid using public terminals located in restricted offices, emergency back corridors, closed wards, or archive areas.
