Request a copy of your medical records

Patients may request a copy of their medical records from Brookhaven Hospital, Alchemilla Hospital, or another Silent Hill Healthcare facility. Medical records may include visit summaries, discharge instructions, diagnostic reports, medication lists, billing-related documentation, and other information maintained as part of your designated medical record.

Requests are reviewed by Health Information Management before records are released. This helps confirm patient identity, protect confidential information, and ensure that the correct facility record is provided.

Note: Medical records requests may take longer during lake fog advisories, townwide siren events, records-room service interruptions, archive retrieval delays, or facility access restrictions. Please do not submit duplicate requests while your original request is under review.

Before you begin

Before submitting a request, gather the information needed to locate your record and verify your identity.

  • Your full legal name as listed on your medical record
  • Your date of birth
  • Your Brookhaven, Alchemilla, or Silent Hill Healthcare Patient ID, if available
  • The facility where you received care
  • The approximate date or date range of treatment
  • The type of records you are requesting
  • A valid email address, mailing address, or preferred delivery method
  • Legal documentation, if requesting records for another patient
Important: If you are requesting records on behalf of a dependent, guardian, estate, or another adult patient, additional authorization may be required before records can be released.
Tip: If you received care at both Brookhaven and Alchemilla, include both facility names in your request. Records from each facility may be stored under separate medical record numbers.

Records you can request

You may request copies of many records maintained by Silent Hill Healthcare, including:

  • After-visit summaries
  • Discharge instructions
  • Medication lists
  • Laboratory and diagnostic test results
  • Imaging reports
  • Emergency department documentation
  • Inpatient admission and discharge records
  • Transfer summaries between Brookhaven and Alchemilla
  • Billing-related medical documentation
  • Provider notes that are eligible for patient release

Some records may already be available in your patient portal. If you only need a recent visit summary, sign in to the portal before submitting a formal records request.

Submit a medical records request

To request a copy of your medical records:

  1. Sign in to the portal for your current or most recent care facility:
  2. Go to Medical Records or Health Information Management.
  3. Select Request a copy of my records.
  4. Choose the facility where you received care.
  5. Enter the date range for the records you need.
  6. Select the record types you are requesting.
  7. Choose your preferred delivery method.
  8. Review the authorization statement.
  9. Select Submit request.
Note: You may be asked to verify your identity before the request is accepted. Verification may include a portal sign-in, one-time code, Patient ID confirmation, or additional documentation.

Delivery options

Depending on the record type and review requirements, records may be delivered through one of the following methods:

Patient portal Best for recent visit summaries, discharge instructions, medication lists, and eligible test results.
Secure email Available for many standard record requests after identity verification is complete.
Mail Available for paper copies. Delivery may be affected by local route closures, fog advisories, or address verification issues.
In-person pickup Available at designated Records Office locations with valid identification.
Tip: Choose portal or secure email delivery if you need records quickly. Paper records and older archived files may take longer to prepare.

Processing times

Processing time depends on the type of records requested, the facility where care was provided, and whether additional review is needed.

Recent portal records Often available immediately or within 24 hours.
Standard records request Usually processed within 3–5 business days after identity verification.
Multi-facility request May take 5–10 business days if Brookhaven and Alchemilla records must be reviewed together.
Archived or restricted records May take longer depending on archive location, clinician review, legal review, or facility access restrictions.

Restricted or specially reviewed records

Some records require additional review before they can be released. This may include:

  • Psychiatric treatment notes
  • Records involving another patient’s private information
  • Records connected to legal holds or active investigations
  • Historical paper files requiring archive retrieval
  • Basement archive files or special ward documentation
  • Records involving guardianship, caregiver access, or minor patients

If a requested record cannot be released through the portal, Health Information Management will provide instructions for next steps when possible.

Important: Do not submit requests for another patient’s records unless you are legally authorized to do so. Unauthorized requests may be denied and may delay future records processing.

FAQ

I cannot find the medical records request form
  • Confirm that you are signed in to the portal for your current or most recent care facility.
  • Look under Medical Records, Documents, or Health Information Management.
  • Check whether your account is still pending identity verification.
  • Contact Patient Accounts & Records if the form does not appear.
My request was denied
  • Confirm that the request is for your own records or that you have legal authorization.
  • Make sure your identity verification information matches your medical record.
  • Upload any requested authorization or guardianship documentation.
  • Contact Health Information Management if you believe the denial was made in error.
I received only part of my record

Some records may be released separately. Billing documentation, diagnostic reports, transfer summaries, older paper files, and restricted notes may have different review timelines.

If the missing information is not available after the expected processing time, submit a follow-up request or contact Health Information Management.

My records are listed under the wrong facility
  • Check whether the record is a transfer summary from another Silent Hill Healthcare facility.
  • Confirm that Brookhaven and Alchemilla records are linked under the correct profile.
  • Review the facility name listed on your discharge or billing paperwork.
  • Contact Patient Accounts & Records if the record appears attached to the wrong patient.
The records request page looks wrong or keeps refreshing
  • Refresh your browser.
  • Clear your browser cache.
  • Try a different browser or device.
  • Wait and try again if the town siren has recently sounded.
  • Avoid using public terminals located in closed wards, basements, or restricted wings.
