View visit summaries in your patient portal

Visit summaries provide an overview of care you received at Brookhaven Hospital, Alchemilla Hospital, or another Silent Hill Healthcare facility. These summaries may include after-visit instructions, diagnosis information, treatment recommendations, care plans, medication changes, referrals, and discharge instructions.

You can view eligible visit summaries in your patient portal after the care team completes documentation and Health Information Management releases the information to your account.

Note: Visit summary availability may be delayed during records-room service interruptions, department review delays, emergency department surges, lake fog advisories, or townwide siren events. If a summary does not appear immediately, check again later before submitting a records request.

Before you begin

To view visit summaries, make sure you have:

  • An active Brookhaven or Alchemilla patient portal account
  • Your portal username and password
  • Access to two-factor authentication, if enabled
  • Your Patient ID, if the portal asks you to verify your profile
  • The approximate visit date or discharge date you want to review
Tip: If you received care at both Brookhaven and Alchemilla, confirm that your records are linked under one profile. Visit summaries may appear under the facility where the care was provided.

Where to find visit summaries

Visit summaries are usually available from the Visits, Medical Records, or Documents area of the patient portal.

  1. Sign in to the portal for your current or most recent care facility:
  2. Select Medical Records, Visits, or Documents.
  3. Choose the facility, if more than one appears.
  4. Select the visit date you want to review.
  5. Open Visit Summary, After-Visit Summary, or Discharge Instructions.
  6. Use Download, Print, or Send securely, if available.
Note: Some portal accounts use separate tabs for Hospital Visits, Emergency Visits, Behavioral Health Visits, and Discharge Documents. Check each area if you do not see the summary right away.

What appears in visit summaries

The information shown depends on the type of care you received and whether the care team has completed documentation.

After-visit summary Usually includes the visit date, facility, reason for visit, care team, follow-up instructions, and general care recommendations.
Diagnosis notes May include diagnosis names, assessment details, clinical impressions, and related provider documentation eligible for patient release.
Care plan May include treatment goals, follow-up appointments, medication guidance, referrals, therapy recommendations, and home care instructions.
Discharge instructions Usually includes instructions for recovery, warning signs, medications, follow-up care, activity limits, and when to seek urgent care.

Care plans and discharge instructions

Care plans and discharge instructions may appear separately from the main visit summary. This is common for emergency department visits, inpatient stays, surgical procedures, behavioral health treatment, and facility transfers.

Depending on your visit, you may see separate documents for:

  • Medication changes or medication reconciliation
  • Follow-up appointments
  • Referral instructions
  • Wound care or post-operative instructions
  • Activity restrictions
  • Safety planning or behavioral health follow-up
  • Transfer instructions between Alchemilla and Brookhaven
  • Return precautions or symptoms that require urgent evaluation
Important: If your discharge instructions say to seek urgent care for worsening symptoms, new symptoms, medication reactions, confusion, severe pain, breathing difficulty, or thoughts of self-harm, contact emergency services or go to the nearest emergency department.

When summaries become available

Visit summaries are usually released after the care team completes documentation and the record is processed by the facility.

Outpatient visit Often available within 24 hours after the visit is closed.
Emergency visit Usually available after discharge documentation is finalized.
Inpatient stay May take 24–72 hours after discharge, depending on documentation and department review.
Behavioral health visit May require additional review before diagnosis notes, therapy summaries, or safety planning documents appear.
Tip: If your visit involved multiple departments, documents may appear at different times. For example, an emergency visit summary may appear before imaging reports, medication records, transfer notes, or discharge planning documents.

FAQ

I do not see my visit summary
  • Confirm that you are signed in to the correct Brookhaven or Alchemilla portal profile.
  • Search by visit date, discharge date, or facility name.
  • Check Documents, Visits, and Medical Records.
  • Allow additional time if the visit was recent or involved multiple departments.
Why is my visit summary or record delayed?

Some visit summaries, diagnosis notes, care plans, lab results, and discharge instructions may not appear right away. Records may need provider sign-off, lab review, department processing, privacy review, or archive retrieval before they are released to the portal.

My diagnosis notes are missing

Diagnosis notes may appear after the provider finalizes documentation. Some notes may be summarized, held for clinical review, or released through a formal medical records request rather than the visit summary page.

My discharge instructions are not attached
  • Check the Discharge Documents or Care Plan area of the visit.
  • Review secure messages from your care team.
  • Check whether the discharge instructions were issued by another department or facility.
  • Contact Health Information Management if the instructions are still missing after 72 hours.
My visit appears under the wrong facility
  • Check whether the visit involved a transfer between Brookhaven and Alchemilla.
  • Confirm that your records are linked under one portal profile.
  • Review the facility listed on your discharge paperwork.
  • Contact Patient Accounts & Records if the visit appears attached to the wrong patient.
The visit page looks wrong or keeps refreshing
  • Refresh your browser.
  • Clear your browser cache.
  • Try a different browser or device.
  • Wait and try again if the town siren has recently sounded.
  • Avoid using public terminals located in closed wards, basements, or restricted wings.
