Request older paper records from Silent Hill facilities
Some Silent Hill Healthcare records were created before the current patient portal and may exist only as paper files, scanned images, microfilm references, legacy index cards, or archived municipal records. These records may not appear in your portal until they are manually located, reviewed, scanned, and attached to the correct patient profile.
Older paper records may include prior Alchemilla Hospital records, archived Brookhaven Hospital files, transfer documentation, historical intake forms, older discharge summaries, or records maintained by Silent Hill municipal health offices before facility systems were consolidated.
Before you begin
Older records can be harder to locate than recent portal records. Providing specific details helps Health Information Management search the correct archive.
- Your full legal name at the time of treatment
- Any previous names, aliases, or name changes used during treatment
- Your date of birth
- Your Brookhaven, Alchemilla, or Silent Hill Healthcare Patient ID, if available
- The approximate year, month, or date range of treatment
- The facility where care was provided
- The type of record you are requesting
- Any discharge paperwork, billing statements, wristband numbers, or transfer forms you still have
- Legal authorization documents, if requesting records for another patient
What counts as an older paper record
An older paper record is any patient record that was not created directly in the current Silent Hill Healthcare electronic medical record system. These files may require manual retrieval before they can be released.
Older records may be stored as:
- Paper charts
- Archived intake packets
- Typed discharge summaries
- Handwritten physician or nursing notes
- Medication administration sheets
- Legacy Alchemilla department logs
- Brookhaven ward files or treatment folders
- Scanned images not yet attached to the portal
- Municipal public health forms or transfer registers
- Microfilm, microfiche, or index-card references
Older records you can request
You may request older records from Silent Hill facilities, including:
- Prior Alchemilla Hospital emergency department records
- Older Alchemilla inpatient or surgical records
- Archived Brookhaven Hospital inpatient treatment records
- Brookhaven behavioral health or therapy documentation eligible for release
- Historical discharge summaries
- Older medication administration records

Some older records may require privacy review, legal review, or clinical review before release, especially if the file includes behavioral health treatment, another patient’s information, guardianship documentation, or restricted facility notes.
Submit an archive retrieval request
To request older paper records from a Silent Hill facility:
- Sign in to your Silent Hill Healthcare patient portal:
- Go to Medical Records or Health Information Management.
- Select Request older paper records or Archive retrieval request.
- Choose the facility or department most likely to hold the record.
- Enter the approximate treatment date or date range.
- Enter any previous names, Patient IDs, ward names, visit numbers, or billing references.
- Select the type of record you need.
- Upload any supporting paperwork, if available.
- Choose your preferred delivery method.
- Review the authorization statement and select Submit request.
Manual retrieval and archive review
Older paper records are not always immediately available to portal staff. Archive retrieval may require several steps before records can be released.

Processing times
Older paper record requests usually take longer than recent portal record requests.

| Municipal archive request | May take 10–20 business days if records must be matched against older municipal indexes. |

FAQ
I do not know which facility has my older records
- Include any facility names, department names, or locations you remember.
- List the approximate year or date range of treatment.
- Include any old billing notices, discharge papers, or transfer documents you still have.
- Health Information Management may search multiple facility indexes if needed.
My older record was not found
Older records may be indexed under a previous name, older medical record number, ward assignment, municipal register number, or treatment year. If your request was not found, submit any additional identifying details you have so staff can search again.
I received only part of an older file
Some archived files may be incomplete, damaged, duplicated across facilities, or separated by department. Health Information Management releases available records that can be identified, reviewed, and approved for release.
My archive request is still pending
- Allow the full processing time for the type of archive request submitted.
- Check secure messages for requests for additional identification or authorization.
- Requests involving older paper files, municipal indexes, or restricted records may take longer.
- Do not submit a duplicate request unless Health Information Management asks you to.
The archive request page looks wrong or keeps refreshing
- Refresh your browser.
- Clear your browser cache.
- Try a different browser or device.
- Wait and try again if the town siren has recently sounded.
- Avoid using public terminals located in closed wards, basements, or restricted archive areas.
