Request Alchemilla Hospital emergency or inpatient records

Patients may request copies of eligible Alchemilla Hospital emergency, surgical, inpatient, and general hospital records through the Silent Hill Healthcare patient portal or by contacting Alchemilla Health Information Management.

Alchemilla records may include emergency department documentation, triage notes, physician assessments, surgical reports, inpatient stay summaries, medication administration records, discharge instructions, and transfer documentation to or from another Silent Hill Healthcare facility.

Note: Alchemilla records requests may take longer during emergency department surges, records-room service interruptions, archive retrieval delays, power interruptions, lake fog advisories, or townwide siren events. Please do not submit duplicate requests while your original request is under review.

Before you begin

Before requesting Alchemilla Hospital records, gather the information needed to locate your hospital chart and confirm your identity.

  • Your full legal name as listed on your Alchemilla record
  • Your date of birth
  • Your Alchemilla Patient ID or medical record number, if available
  • The approximate date or date range of treatment
  • Whether the care was emergency, surgical, inpatient, outpatient follow-up, or transfer-related
  • The specific records you are requesting
  • A valid email address, mailing address, or preferred delivery method
  • Legal authorization documents, if requesting records for another patient
Important: If you are requesting records on behalf of a dependent, guardian, estate, or another adult patient, Alchemilla may require signed authorization, guardianship paperwork, or other legal documentation before records can be released.
Tip: If you were transferred from Alchemilla Hospital to Brookhaven Hospital, include both facility names and any transfer paperwork. Emergency department records and transfer summaries may be stored separately from inpatient or follow-up records.

Alchemilla records you can request

You may request copies of many Alchemilla Hospital records, including:

  • Emergency department visit summaries
  • Triage notes and nursing assessments
  • Physician assessments and provider notes eligible for release
  • Laboratory and diagnostic test results
  • Imaging reports
  • Medication administration records
  • Inpatient admission and discharge documentation
  • Surgical notes and operative reports
  • Anesthesia documentation, when available for release
  • Discharge summaries and discharge instructions
  • Referral and follow-up care instructions
  • Transfer summaries between Alchemilla and Brookhaven

Some recent discharge instructions, lab results, and visit summaries may already be available in your patient portal. If you only need a recent emergency visit summary or discharge instruction sheet, check the portal before submitting a formal records request.

Emergency department records

Emergency department records are created when you receive urgent or unscheduled care through Alchemilla Hospital. These records may include triage documentation, nursing notes, provider assessments, diagnostic testing, administered medications, discharge instructions, and transfer recommendations.

When requesting emergency records, include:

  • The date of the emergency visit
  • The approximate arrival time, if known
  • Whether you arrived by ambulance, walk-in intake, police escort, or facility transfer
  • Any discharge paperwork or visit number you received
  • Whether you were discharged, admitted, transferred, or asked to return for follow-up care
Note: Emergency records may be split across different departments if your visit included diagnostic imaging, laboratory testing, surgical consultation, psychiatric consultation, or transfer to Brookhaven Hospital.

Surgical and inpatient records

Surgical and inpatient records may require additional review because they often include notes from multiple care teams, medication administration details, procedure documentation, and discharge planning materials.

Records that may require additional review include:

  • Operative reports
  • Pre-operative and post-operative notes
  • Anesthesia records
  • Inpatient medication administration records
  • Nursing shift notes
  • Consultation notes from another department or facility
  • Care coordination and discharge planning records
  • Records involving another patient’s private information
  • Older paper files stored in Alchemilla archives
Important: Some surgical, inpatient, or special ward records may be released in part, summarized, or held for additional review if they contain restricted information, legal holds, or details about another patient.

Submit an Alchemilla records request

To request Alchemilla Hospital emergency or inpatient records:

  1. Sign in to the Alchemilla patient portal:
  2. Go to Medical Records or Health Information Management.
  3. Select Request Alchemilla hospital records.
  4. Enter the date range for the records you need.
  5. Select the type of care:
    • Emergency department visit
    • Inpatient stay
    • Surgical procedure
    • Medication administration history
    • Diagnostic testing or imaging
    • Discharge summary
    • Transfer documentation
  6. Choose your preferred delivery method.
  7. Upload supporting authorization documents if requested.
  8. Review the authorization statement.
  9. Select Submit request.
Tip: Be specific when choosing record types. A request for “all hospital records” may take longer than a request for an emergency visit summary, operative report, medication administration record, or discharge summary.

Processing and review

Alchemilla Health Information Management reviews each request before release. Processing time depends on the record type, treatment date, and whether additional departmental review is required.

Emergency visit summary Often available in the portal within 24 hours after discharge.
Discharge summary Usually processed within 3–5 business days after identity verification.
Surgical or operative report Usually processed within 5–10 business days depending on procedure documentation and review needs.
Full inpatient chart May take longer depending on length of stay, number of departments involved, and chart review requirements.
Archived Alchemilla records May take longer depending on archive location, paper file condition, or facility access restrictions.
Note: Some records may be released in stages. Emergency documentation, surgical notes, medication administration records, diagnostic reports, and transfer summaries may have separate review timelines.

FAQ

I cannot find my Alchemilla emergency record
  • Confirm that you are signed in to the Alchemilla portal.
  • Search by emergency visit date, discharge date, or visit number.
  • Check whether the visit resulted in an inpatient admission or transfer record.
  • Contact Health Information Management if the record does not appear.
My surgical notes were not included

Some surgical notes may require additional departmental review before release. Operative reports, anesthesia records, and post-operative notes may be released separately depending on the procedure and chart review requirements.

My inpatient record looks incomplete
  • Check whether the missing information belongs to a separate department, such as imaging, surgery, pharmacy, or laboratory services.
  • Confirm the date range used in your records request.
  • Review discharge summaries separately from medication administration records.
  • Contact Health Information Management if a department note appears to be missing.
My request is still pending
  • Allow the full processing time for the record type requested.
  • Check secure messages for requests for additional verification or authorization.
  • Requests involving surgery, full inpatient charts, older paper files, or restricted records may take longer.
  • Do not submit a duplicate request unless Health Information Management asks you to.
The Alchemilla records page looks wrong or keeps refreshing
  • Refresh your browser.
  • Clear your browser cache.
  • Try a different browser or device.
  • Wait and try again if the town siren has recently sounded.
  • Avoid using public terminals located in closed wards, basements, or restricted wings.
