Request Brookhaven Hospital treatment records

Patients may request copies of eligible Brookhaven Hospital treatment records through the Silent Hill Healthcare patient portal or by contacting Brookhaven Health Information Management. Brookhaven records may include inpatient treatment summaries, outpatient visit documentation, behavioral health records, medication history, therapy documentation, and discharge summaries.

Because Brookhaven Hospital provides psychiatric and behavioral health services, some records may require additional clinical review before release. This helps protect patient privacy, confirm the correct medical record, and ensure that sensitive documentation is released through the appropriate process.

Note: Brookhaven treatment record requests may take longer during records-room service interruptions, archive retrieval delays, transfer file reviews, lake fog advisories, or townwide siren events. Please do not submit duplicate requests while your original request is under review.

Before you begin

Before requesting Brookhaven treatment records, gather the information needed to identify your Brookhaven medical record and treatment episode.

  • Your full legal name as listed on your Brookhaven record
  • Your date of birth
  • Your Brookhaven Patient ID or medical record number, if available
  • The approximate date or date range of treatment
  • Whether the care was inpatient, outpatient, emergency transfer, or follow-up treatment
  • The specific record types you are requesting
  • A valid email address, mailing address, or preferred delivery method
  • Legal authorization documents, if requesting records for another patient
Important: If you are requesting records on behalf of a dependent, guardian, estate, or another adult patient, Brookhaven may require signed authorization, guardianship paperwork, or other legal documentation before records can be released.
Tip: If you were transferred from Alchemilla Hospital to Brookhaven, include both facility names and any transfer paperwork. Transfer summaries may be stored separately from your Brookhaven inpatient or outpatient record.

Brookhaven records you can request

You may request copies of many Brookhaven treatment records, including:

  • Inpatient admission records
  • Discharge summaries and discharge instructions
  • Outpatient behavioral health visit summaries
  • Medication administration records
  • Medication history and active medication lists
  • Care team notes eligible for patient release
  • Group therapy attendance documentation, when available
  • Individual therapy summaries, when eligible for release
  • Treatment plan summaries
  • Safety planning documentation
  • Referral and follow-up care instructions
  • Transfer summaries between Brookhaven and Alchemilla

Some recent visit summaries and medication lists may already be available in your patient portal. If you only need a recent discharge summary or appointment summary, check the portal before submitting a formal records request.

Behavioral health and therapy records

Behavioral health records may contain sensitive clinical information. Brookhaven reviews these requests carefully before release.

Records that may require additional review include:

  • Psychiatric evaluation notes
  • Individual therapy progress notes
  • Group therapy documentation
  • Risk assessments and safety planning notes
  • Medication adjustment documentation
  • Records involving another patient’s private information
  • Records connected to legal holds, guardianship review, or active investigations
  • Older paper files stored in Brookhaven archives
Note: A request for therapy records does not always include psychotherapy process notes. Some notes may be withheld, summarized, or released only after additional clinical review.
Important: Do not request another patient’s behavioral health records unless you are legally authorized to do so. Unauthorized requests may be denied and may delay future records processing.

Submit a Brookhaven records request

To request Brookhaven Hospital treatment records:

  1. Sign in to the Brookhaven patient portal:
  2. Go to Medical Records or Health Information Management.
  3. Select Request Brookhaven treatment records.
  4. Enter the date range for the treatment records you need.
  5. Select the type of care:
    • Inpatient admission
    • Outpatient visit
    • Behavioral health treatment
    • Medication history
    • Discharge summary
    • Transfer documentation
  6. Choose your preferred delivery method.
  7. Upload supporting authorization documents if requested.
  8. Review the authorization statement.
  9. Select Submit request.
Tip: Be specific when choosing record types. A request for “all records” may take longer than a request for a discharge summary, medication history, or treatment plan summary.

Processing and clinical review

Brookhaven Health Information Management reviews each request before release. Processing time depends on the record type, treatment date, and whether clinical review is required.

Recent discharge summary Often available in the portal within 24 hours after discharge.
Medication history Usually processed within 3–5 business days after identity verification.
Inpatient or outpatient chart Usually processed within 5–10 business days depending on chart length and review requirements.
Behavioral health or therapy records May require additional clinical review before release and may take longer.
Archived Brookhaven records May take longer depending on archive location, paper file condition, or facility access restrictions.
Note: Some records may be released in stages. Discharge summaries, medication lists, therapy documentation, and transfer notes may have separate review timelines.

I cannot find my Brookhaven treatment records
  • Confirm that you are signed in to the Brookhaven portal.
  • Check whether the records are listed under a linked Alchemilla transfer profile.
  • Search by treatment date, discharge date, or care type.

My therapy notes were not included

Some therapy notes may require additional clinical review, may be summarized, or may not be released through the portal. If you requested therapy records and only received part of your file, contact Brookhaven Health Information Management for next steps.

My medication history looks incomplete
  • Check whether the medication was prescribed at Brookhaven or another facility.
  • Review your discharge medication list separately from your active medication list.
  • Confirm the date range used in your records request.
  • Contact Health Information Management if a medication order appears to be missing.
My request is still pending
  • Allow the full processing time for the record type requested.
  • Check secure messages for requests for additional verification or authorization.
  • Requests involving behavioral health, older paper files, or restricted records may take longer.
  • Do not submit a duplicate request unless Health Information Management asks you to.
The Brookhaven records page looks wrong or keeps refreshing
  • Refresh your browser.
  • Clear your browser cache.
  • Try a different browser or device.
  • Wait and try again if the town siren has recently sounded.
  • Avoid using public terminals located in closed wards, basements, or restricted wings.
