Request imaging, X-rays, or diagnostic reports

You may be able to view many imaging and diagnostic reports in the Silent Hill Health portal after they are finalized. This may include X-ray, CT, MRI, ultrasound, mammogram, EKG, procedure-related diagnostic reports, or other written interpretations.

The written report is not always the same as the actual image files. Another provider may need the written report, the image files, or both. If the report or image files are not available directly in the portal, use Request Records, Radiology Images, or the medical records process.

Open Tests & Results or Imaging, select the report, then check whether you need View Report, View Images, Request Image Copy, or Request Records.

Reports vs. image files

Imaging records usually have two parts: the written report and the image files. The report is the written interpretation. The image files are the actual pictures or scan data from the imaging exam.

Some appointments only require the written report. Specialists, surgeons, outside radiologists, or second-opinion providers may need the image files too. Ask the receiving office what format they need before submitting a request.

Find a written report in the portal

Start in Tests & Results. If the imaging or diagnostic report was completed during an emergency visit, hospital stay, or Brookhaven care episode, it may appear under Hospital Results, Imaging, Documents, or the related visit.

Step by step

  1. Sign in to the Silent Hill Health portal.
  2. Open Tests & Results or Imaging.
  3. Search by exam type, body area, date, facility, or ordering provider.
  4. Clear date, provider, and facility filters if you do not see the report.
  5. Choose All Silent Hill Health if the report could be under Alchemilla, Brookhaven, or an outside result source.
  6. Open the report and check status, date, facility, and whether an addendum or corrected version exists.

Request image files

Image files may not appear automatically with the written report. If another clinician needs the actual images, use View Images, Request Image Copy, Radiology Images, or Request Records.

Image requests may take longer than downloading a written report. The files may need to be prepared for a secure download, sent to an outside provider, copied to a disc, or released through medical records.

Request image files when

  • A specialist asks to review the actual scan.
  • You are getting a second opinion.
  • An outside surgeon, radiologist, or clinic requests images.
  • The receiving office says the written report is not enough.
  • You need images for comparison with future studies.

Include in your request

  • Imaging type, such as X-ray, CT, MRI, ultrasound, or mammogram.
  • Body area scanned.
  • Exam date or date range.
  • Facility where imaging was performed.
  • Where the images should be sent or how you want to receive them.

Discs or downloads

Some image requests are delivered through secure download. Others may be copied to a disc or sent through a radiology image-sharing system. Ask the receiving provider which format they can accept before choosing a delivery method.

Secure download You or an outside provider can use an electronic image link. Expiration date, access code, file size, and whether the provider can open it.
Disc The receiving provider specifically requests a CD or DVD. Pickup or mailing time, address, and whether the disc includes the report too.
Direct provider sharing Another clinic can receive images electronically from Silent Hill Health. Provider name, facility, address, fax, secure email, or image-sharing destination.
Portal viewer You only need to view the images yourself inside the portal. Whether the viewer lets you download or only view images.

Before requesting a disc: Ask the receiving office whether they still accept discs. Many offices prefer secure electronic image sharing.

Share with an outside provider

If an outside provider needs your imaging or diagnostic records, ask exactly what they need: the written report, the image files, both, or a full medical records release. A portal screenshot may not include enough information for clinical review.

You may be able to share through connected records, direct electronic sharing, secure download, a one-time share tool, or a formal records authorization.

Ask the outside provider:

  • Do you need the written report, the image files, or both?
  • Do you accept secure electronic image sharing?
  • Do you need a disc or physical copy?
  • What facility name, department, fax, address, or secure email should Silent Hill Health use?
  • Do you need the records before my appointment date?

If your outside provider says a portal download is not enough, use Request Brookhaven-related lab or diagnostic records for Brookhaven-related records, or use the general Request Records path for Alchemilla and other Silent Hill Health records.

Use medical records requests

Use the medical records process when you need an official copy, image files, older records, a full diagnostic report, a report that is not visible in the portal, or records sent to another provider or organization.

Records request step by step

  1. Open Records or Request Records.
  2. Choose the facility or select All Silent Hill Health if you are unsure.
  3. Choose the record type: Imaging Report, Radiology Images, Diagnostic Report, or Selected Records.
  4. Enter the exam date, body area, and test type if you know them.
  5. Choose delivery: portal, secure download, mail, disc, outside provider, or authorized representative.
  6. Submit any required identity, proxy, caregiver, or authorization information.

If the report is not available

A missing imaging or diagnostic report may still be processing, waiting for review, hidden by filters, linked to another facility, or available only through records. The next step depends on what you see in the portal.

Preliminary report An early report may be available before final review. Wait for final report if the receiving provider needs final results.
No imaging result found The report may be filtered, attached to another visit, or under another facility. Clear filters and choose All Silent Hill Health.
Outside imaging center Silent Hill Health may not have received the report or images yet. Contact the outside center or ask the ordering provider whether records were received.
Request Through Records The report or image files may not release automatically in portal view. Use Request Records or contact Medical Records.

Brookhaven-related imaging or diagnostic records

Some imaging or diagnostic records tied to a Brookhaven Hospital stay or behavioral health care episode may have additional privacy, proxy, provider-review, or records-release steps. Linking Brookhaven and Alchemilla records does not always make every Brookhaven-related result visible automatically.

If the portal shows Brookhaven Review, Sensitive Result, Proxy Access Limited, or Request Through Records, use Brookhaven Records, Health Information Management, or the Brookhaven care team based on what you need.

Privacy note: A support person may help you request records, but Brookhaven-related records may still require patient permission, proxy authorization, records review, or privacy review before details can be shared.

Is the imaging report the same as the images?

No. The report is the written interpretation. The images are the actual scan files. Some providers need only the report, while others need both the report and image files.

Where do I find my X-ray, CT, MRI, or ultrasound report?

Start in Tests & Results or Imaging. If you do not see it, clear filters, choose All Silent Hill Health, and check Hospital Results or the related visit.

How do I get the actual images?

Use View Images, Request Image Copy, Radiology Images, or Request Records. Image files may be sent by secure download, image-sharing system, or disc depending on the request.

Can Silent Hill Health send images to an outside provider?
