Send test results to another provider

Send test results to another provider

You can share many Silent Hill Health test results with another provider for referrals, second opinions, outside specialist visits, follow-up care, surgery planning, medication review, or records transfer. Depending on what the receiving provider needs, you may be able to download the result yourself, share it through connected records, use a one-time sharing tool, or submit a formal records request.

Before sending anything, ask the receiving provider whether they need a lab result, written imaging report, diagnostic report, actual image files, full record set, or direct release from Silent Hill Health. A screenshot is often not enough for clinical review.

Best first step

Open the result, confirm the test name and date, then choose Share With Provider, Download PDF, Request Records, or Request Image Copy depending on what the receiving provider asked for.

Quick summary

  • Ask the receiving provider exactly what they need before sending results.
  • Use a portal download for quick copies when the provider accepts patient-provided records.
  • Use Request Records when an official copy or direct release is needed.
  • Imaging reports and actual image files may need separate requests.
  • Brookhaven-related or sensitive results may require additional authorization or privacy review.
  • Include the provider’s name, facility, contact information, test name, date, and deadline.

Before you send results

Results can be shared in different ways. Some providers can accept a patient-downloaded PDF. Others need a direct records release, electronic transfer, secure image link, or official copy from Silent Hill Health.

Before starting a request, confirm what the receiving provider needs and whether the result is final. If the result is preliminary, corrected, addended, restricted, or still pending provider review, the receiving provider may ask for the final version later.

Ask before sending Why it matters
Do you need the written result, images, or both? Imaging reports and image files are often separate records.
Do you accept a patient-downloaded PDF? Some offices require direct release from Silent Hill Health.
Do you need the record before an appointment date? Records requests and image-copy requests may take processing time.
Which department should receive it? Large specialist offices may have separate referral, records, and imaging teams.
Do you need an authorization form? Some releases, especially Brookhaven-related or third-party requests, may need authorization.

Choose a sharing method

The right method depends on what the other provider needs. Use the simplest method that meets the receiving provider’s requirements.

Sharing method Use this when Where to start
Download or print from portal The provider accepts a patient copy for review. Tests & Results or Documents.
Share with provider The provider can receive a secure portal share or connected-record view. Share With Provider, Sharing Hub, or Connected Records.
Request Records An official copy, direct release, full record set, or third-party delivery is needed. Records or Request Records.
Request image copy The provider needs actual X-ray, CT, MRI, ultrasound, or other image files. Imaging, Radiology Images, or Request Image Copy.
Authorization form Records are being released to a third party, caregiver, program, attorney, school, or outside organization. Request Records, Release of Information, or Health Information Management.

For downloading results yourself, review Download or print lab results from your portal. For imaging files or X-rays, review Request imaging, X-rays, or diagnostic reports.

Information needed

For referrals

If your provider referred you to another clinician, the referral may include some records automatically. However, the specialist may still ask for specific results, imaging, reports, medication lists, or outside records before they can schedule or review your case.

Referral need What to send or confirm
Specialist review Recent results, relevant imaging reports, medication list, and provider notes if requested.
Referral authorization Referral number, ordering provider, insurance details, and receiving clinic information.
Imaging-based referral Written report plus actual image files if the specialist asks for them.
Behavioral health or Brookhaven referral Ask whether special authorization or Brookhaven records review is needed.

For help scheduling follow-up after a result, review Schedule follow-up care after lab or imaging results.

For outside specialists

Outside specialists may not have direct access to your Silent Hill Health portal. Ask their office how they want records sent before submitting your request.

Some offices accept a patient-uploaded PDF. Others need records faxed or sent electronically from Silent Hill Health. For imaging, they may need image files sent through a secure image exchange or copied to a disc.

Ask the outside specialist’s office:

  • Do you need the written report, actual image files, or both?
  • Do you accept patient-uploaded PDFs?
  • Should Silent Hill Health send records directly?
  • What fax, secure email, address, or electronic destination should be used?
  • Do you need a referral number, authorization number, or insurance approval?
  • When do you need the records by?

For follow-up care

If you are sharing results for follow-up care, send the records that explain the current question. This may be the most recent result, a trend of several results, an imaging report, medication monitoring labs, discharge testing, or records connected to an emergency visit or hospital stay.

Common follow-up records

  • Recent lab results.
  • Imaging reports and image files if requested.
  • Diagnostic reports.
  • Discharge summary or after-visit summary.
  • Medication list and medication monitoring results.

Ask your care team if:

  • You do not know which result should be sent.
  • The result is preliminary or corrected.
  • Repeat testing is planned soon.
  • You need records before a procedure or surgery.
  • A Brookhaven-related result may need privacy review.

If you need help deciding what a result means before sharing it, review Ask your care team about a lab or imaging result.

Lab results vs. imaging files

Lab results, imaging reports, and image files are different record types. The receiving provider may need one or more of them.

Record type What it is How to share
Lab result Blood, urine, culture, medication level, or other lab report. Download PDF, print, Share With Provider, or Request Records.
Imaging report Written interpretation from radiology or imaging specialist. View Report, Download PDF, Share With Provider, or Request Records.
Image files Actual X-ray, CT, MRI, ultrasound, mammogram, or scan images. Request Image Copy, Radiology Images, secure download, disc, or direct provider sharing.
Diagnostic report Written report from EKG, procedure-related testing, or other diagnostic service. Tests & Results, Documents, or Request Records.

For more detail on reports and image files, review Request imaging, X-rays, or diagnostic reports.

Brookhaven-related or sensitive results

Some Brookhaven-related results may have additional privacy, proxy, safety, or authorization review before they can be shared. This can include behavioral health-related results, substance-related records, safety-related documentation, minor or dependent account records, and results marked sensitive or restricted.

If the result shows Brookhaven Review, Sensitive Result, Proxy Access Limited, or Request Through Records, ask whether a separate authorization or Brookhaven Records review is required before sending it.

Privacy note: Linked Alchemilla and Brookhaven records can make results easier to find, but linking does not remove Brookhaven privacy rules or proxy-access limits.

For Brookhaven-specific guidance, review Understand Brookhaven test result privacy, Request Brookhaven-related lab or diagnostic records, and Link Brookhaven and Alchemilla results under one profile.

After you submit a share or records request

After submitting a request, watch the portal for request status updates. The request may need identity verification, authorization review, record search, Brookhaven privacy review, image-copy processing, or confirmation that the outside provider received the files.

Request status What it may mean
Submitted The request was received but has not been completed yet.
Needs Information Silent Hill Health needs more provider details, authorization, or identity information.
Processing Records are being located, prepared, or sent.
Brookhaven Review The request includes Brookhaven-related or sensitive information that needs review.
Sent or Released The records were sent to the approved destination or released to the requested account.

FAQ

Can I send results myself from the portal?

Sometimes. If the result is available, you may be able to download, print, or share it. Ask the receiving provider whether they accept patient-provided copies or need an official release from Silent Hill Health.

What information do I need for the receiving provider?

Include the provider’s name, facility, department or specialty, fax number, secure email, mailing address, electronic sharing destination, and any deadline or appointment date.

Do I need a records request or a referral?

They are different. A referral is a clinical or insurance request to see another provider. A records request sends health information. Some specialist visits need both.

Can Silent Hill Health send imaging files to another provider?

Often, yes. Use Request Image Copy, Radiology Images, or Request Records. Ask whether the outside provider wants a secure download, direct electronic sharing, or a disc.

What if I need results sent before an upcoming appointment?

Submit the request as early as possible and include the appointment date. If the appointment is soon, call the receiving office and Silent Hill Health records support to confirm the best delivery method.

Can a caregiver send records for me?

A caregiver may be able to help if they have proxy access, caregiver access, legal authority, or a signed authorization. Sensitive or Brookhaven-related records may still need additional review before release.

Who explains what the result means?

Records staff can help send results. The ordering provider, specialist, or care team should explain what the result means and what follow-up is needed.
