Schedule an Alchemilla Hospital appointment

Use this article to request an appointment with Alchemilla Hospital. Alchemilla scheduling may include outpatient medical visits, emergency department follow-up, inpatient discharge follow-up, lab work, imaging, diagnostic testing, procedure follow-up, and specialist referral appointments.

Some Alchemilla appointments can be scheduled directly through the portal. Others may require provider review, discharge review, insurance verification, referral approval, lab or imaging order review, or records matching before an appointment time is confirmed.

Note: Alchemilla appointment availability may change during emergency department surges, procedure schedule changes, imaging equipment downtime, lab processing delays, lake fog advisories, road access restrictions near Old Silent Hill, or townwide siren events.

 

Alchemilla appointments you can request

Choose the appointment reason that best matches your medical need. Scheduling Services may update the request type after review.

Outpatient medical visit For routine medical concerns, follow-up questions, medication review, symptom review, or non-emergency outpatient care.
Emergency department follow-up For follow-up after an Alchemilla emergency visit, observation stay, discharge instruction, or emergency-provider recommendation.
Inpatient discharge follow-up For follow-up after a hospital admission, short-stay observation, surgery, inpatient treatment, or discharge planning.
Lab work For bloodwork, specimen collection, medication monitoring, pre-visit testing, or tests ordered after emergency or inpatient care.
Imaging or diagnostic testing For X-rays, diagnostic imaging, CT review, ultrasound, radiology appointments, or testing ordered by an Alchemilla provider.
Procedure follow-up For follow-up after a procedure, dressing check, medication instruction, result review, or provider-directed return visit.
Tip: If your appointment is related to a recent emergency visit, include the emergency visit date and any discharge instructions in your request.

 

Before you begin

Before requesting an Alchemilla appointment, gather any details that may help Scheduling Services route the request correctly.

  • Your full legal name and date of birth
  • Your Silent Hill Health Patient ID, if available
  • Your preferred phone number, email address, or portal message preference
  • The reason for the appointment
  • Recent Alchemilla emergency visit, discharge, or inpatient information
  • Any lab, imaging, diagnostic, or procedure orders
  • Referral details, if another provider told you to schedule at Alchemilla
  • Insurance information, if coverage review may be needed
  • Current medications, allergies, or preparation instructions, if relevant
  • Guardian, caregiver, or authorized-access information, if someone else helps manage your care

Request an Alchemilla appointment through the portal

To request an Alchemilla Hospital appointment:

  1. Sign in to the Alchemilla patient portal:
  2. Go to Appointments, Alchemilla Scheduling, or Request care.
  3. Select the appointment reason that best matches your need.
  4. Choose Alchemilla Hospital as the facility.
  5. Enter your preferred dates, times, callback number, and contact method.
  6. Add details about recent emergency care, discharge instructions, test orders, procedure follow-up, or provider instructions.
  7. Upload referral documents, outside records, insurance cards, lab orders, imaging orders, or discharge paperwork if requested.
  8. Review the request summary and select Submit Alchemilla appointment request.

Emergency follow-up and discharge review

If you were seen in the Alchemilla emergency department, your follow-up appointment may depend on your discharge instructions, diagnosis, test results, provider notes, or whether you were referred to another department.

Emergency follow-up may be needed after:

  • Emergency department evaluation
  • Observation stay or short-stay monitoring
  • Inpatient admission after emergency care
  • Procedure, wound care, or medication instruction
  • Lab or imaging follow-up
  • Referral to a specialist or outpatient clinic
  • Behavioral health referral to Brookhaven after medical clearance
Tip: Include the discharge date and any follow-up timeframe listed in your discharge paperwork, such as “follow up within 3 days” or “schedule after lab results are available.”

 

Labs, imaging, and diagnostic testing

Alchemilla may require an active order before lab work, imaging, or diagnostic testing can be scheduled. If an order is missing, unclear, expired, or attached to the wrong profile, Scheduling Services may need additional review.

Your request may be reviewed when it involves:

  • Bloodwork or specimen collection before an upcoming visit
  • X-rays or diagnostic imaging
  • CT, ultrasound, or other radiology appointments
  • Testing ordered after an emergency visit
  • Testing needed before a procedure
  • Medication monitoring or follow-up lab review
  • Outside orders that need Alchemilla department approval

Why some requests need review

Some Alchemilla appointments cannot be confirmed immediately. Scheduling Services may need to review your request before offering a time.

Review may be needed when:

  • The appointment requires a provider order, referral, or insurance authorization
  • A provider must review recent emergency records, labs, imaging, or discharge notes
  • You were transferred between Alchemilla and Brookhaven
  • Your records are delayed, restricted, archived, or not linked between facilities
  • The appointment involves a minor, dependent, guardian, or authorized user
  • A procedure requires preparation instructions before scheduling
  • The requested department has limited availability or facility access restrictions
Tip: Check your portal messages after submitting the request. Alchemilla Scheduling may ask for more information before confirming the appointment.

 

Urgent or emergency symptoms

Alchemilla appointment scheduling is not emergency care. Do not wait for a portal response if you may need immediate medical attention.

Go to the Alchemilla Hospital emergency department or contact local emergency services if your symptoms are severe, sudden, worsening, or unsafe to wait.

Important: Seek emergency care for severe injury, trouble breathing, chest pain, signs of stroke, loss of consciousness, uncontrolled bleeding, severe allergic reaction, or any condition that cannot safely wait for scheduling review.

I was told to follow up after an emergency visit

I cannot schedule lab or imaging testing

Lab and imaging appointments may require an active order before scheduling. If the order is missing, expired, or attached to the wrong profile, Scheduling Services may need to review the request before confirming a time.

My care involved both Alchemilla and Brookhaven

Include both facility names in your request. If records are not linked, Scheduling Services may need Patient Accounts & Records to review the profiles before confirming care.

The scheduling page looks wrong or keeps refreshing
  • Refresh your browser.
  • Clear your browser cache.
  • Try a different browser or device.
  • Check that you are signed in to the correct patient profile.
  • Wait and try again if the town siren has recently sounded.
  • Avoid using public terminals located in restricted offices, emergency back corridors, or archive areas.
