Schedule a specialist referral appointment

Use this article to request a specialist referral appointment with Silent Hill Health. Specialist appointments may be requested after a primary care visit, Alchemilla emergency visit, inpatient discharge, Brookhaven behavioral health review, lab result, imaging result, or outside provider recommendation.

Some specialist appointments cannot be scheduled immediately. Scheduling Services may need to confirm the referral, review medical records, verify insurance authorization, match records between Brookhaven and Alchemilla, or ask for additional information before offering an appointment time.

Note: Specialist availability may change during provider review periods, department closures, referral backlogs, records-room service interruptions, lake fog advisories, or townwide siren events.

 

Specialist referrals you can schedule

Specialist appointment options depend on the reason for referral, facility, insurance requirements, and whether supporting records are available.

Medical specialist referral For referrals to medical specialists after outpatient care, emergency care, inpatient discharge, test results, or provider review.
Diagnostic follow-up referral For specialist review after lab work, imaging, X-rays, diagnostic reports, or procedure results.
Emergency department referral For follow-up recommended by the Alchemilla emergency department, observation unit, or discharge team.
Procedure or surgery follow-up For specialist care after a procedure, surgical consult, post-procedure review, or provider-directed follow-up.
Brookhaven-related referral For referrals connected to Brookhaven behavioral health care, medication monitoring, post-discharge planning, or care coordination.
Tip: If the referral came from an Alchemilla emergency visit or Brookhaven care team, include the visit date and facility name in your request.

 

Before you begin

Before requesting a specialist appointment, gather the details Scheduling Services may need.

  • Your full legal name and date of birth
  • Your Silent Hill Health Patient ID, if available
  • The specialist type or department you were told to schedule with
  • The provider, clinic, hospital, or department that made the referral
  • Referral order, referral number, or authorization number, if available
  • Recent visit, emergency, discharge, lab, imaging, or procedure information
  • Outside records, if the referral came from a non-Silent Hill Health provider
  • Insurance information, if authorization may be needed
  • Preferred dates, times, facility, and contact method
  • Guardian, caregiver, or authorized-access information, if someone else helps manage your care
Important: Do not wait for a specialist referral appointment if your symptoms are severe, sudden, worsening, or unsafe to wait. Go to the Alchemilla Hospital emergency department or contact local emergency services.

 

Request a specialist appointment through the portal

To request a specialist referral appointment:

  1. Sign in to your Silent Hill Health patient portal:
  2. Go to Appointments, Referrals, Specialist scheduling, or Request care.
  3. Select the specialist type or referral reason that best matches your request.
  4. Choose the facility or department listed on your referral, if available.
  5. Enter the referring provider, referral date, referral number, or authorization number if shown.
  6. Add details about recent care, discharge instructions, test results, procedure follow-up, or provider instructions.
  7. Upload referral documents, outside records, insurance information, lab results, imaging reports, or discharge paperwork if requested.
  8. Enter your preferred dates, times, phone number, and contact method.
  9. Review the request and select Submit referral appointment request.
Note: If you are not sure which facility should receive the request, choose Silent Hill Health Scheduling Services and include the referral source in your notes.

 

Referral review and insurance authorization

Some specialist appointments require review before they can be scheduled. This helps confirm that the correct department, provider, records, and insurance authorization are in place.

Review may be needed when:

  • The specialist requires a provider referral before scheduling
  • Insurance authorization or prior approval is required
  • The referral came from an outside provider
  • The referral was created after an Alchemilla emergency visit
  • Recent lab, imaging, diagnostic, or procedure results must be reviewed first
  • Brookhaven and Alchemilla records need to be linked before scheduling
  • The request involves a minor, dependent, guardian, or authorized user
  • The department has limited appointment availability or facility access restrictions
Tip: Check your portal messages after submitting the request. Scheduling Services may ask for a referral copy, insurance information, outside records, or additional visit details before confirming the appointment.

 

Records needed before scheduling

Specialist departments often need supporting records before they can accept or schedule a referral. Missing or delayed records may slow down appointment confirmation.

Supporting records may include:

  • Referral order or provider note
  • Alchemilla emergency or inpatient records
  • Brookhaven treatment or discharge records
  • Lab results
  • Imaging, X-rays, or diagnostic reports
  • Procedure notes or discharge instructions
  • Outside provider records
  • Insurance authorization or claim information
  • Medical release forms, if records must be requested from outside Silent Hill Health

Brookhaven, Alchemilla, and facility routing

Specialist referrals may be routed differently depending on where the referral started and what type of care is needed.

Alchemilla referral Usually used for medical, emergency follow-up, outpatient, procedure, lab, imaging, or diagnostic-related specialist care.
Brookhaven referral Usually used for behavioral health, medication monitoring, post-discharge care coordination, or specialist review connected to Brookhaven treatment.
Shared-care referral Used when the referral depends on records from both facilities, such as an Alchemilla emergency visit followed by Brookhaven treatment review.
Tip: If your care involved more than one facility, include both facility names and the approximate dates of care. This helps Scheduling Services locate the correct records.

FAQ

My referral appointment request is still pending

Specialist referrals may require provider review, insurance authorization, department approval, outside record review, or records matching before scheduling. Check your portal messages for requests for more information.

I cannot find my referral in the portal

The referral may still be processing, may have been sent to a different department, or may be attached to another patient profile. Include the referring provider, referral date, and facility name when contacting Scheduling Services.

My insurance authorization is not approved yet

Some specialist appointments cannot be scheduled until insurance authorization is approved. Scheduling Services may contact you through the portal if your insurance company needs more information.

My referral depends on records from another hospital or provider

Submit a medical release form or upload outside records if requested. Specialist scheduling may be delayed until the records are received and reviewed.

The referrals page looks wrong or keeps refreshing
  • Refresh your browser.
  • Clear your browser cache.
  • Try a different browser or device.
  • Check that you are signed in to the correct patient profile.
  • Wait and try again if the town siren has recently sounded.
  • Avoid using public terminals located in restricted offices, emergency back corridors, closed wards, or archive areas.
