Schedule an appointment with Silent Hill Health

Use this article as a starting point for scheduling care with Silent Hill Health. Appointment requests can be submitted through the patient portal and routed to the appropriate facility, department, or scheduling team.

Brookhaven Hospital and Alchemilla Hospital schedule different types of care. Choosing the correct facility helps prevent delays, duplicate requests, and appointment routing issues.

Note: Appointment availability may change during provider review periods, department closures, lake fog advisories, road access restrictions near Toluca Lake, or townwide siren events.

 

Choose the correct facility

Select the facility that best matches the reason for your appointment.

Brookhaven Hospital Choose Brookhaven for behavioral health intake, therapy-related care, psychiatric evaluation, medication management, crisis follow-up, post-discharge behavioral health care, or Brookhaven treatment follow-up.
Alchemilla Hospital Choose Alchemilla for medical appointments, emergency department follow-up, outpatient visits, inpatient discharge follow-up, labs, imaging, diagnostic testing, procedures, or specialist referrals.
Silent Hill Health Scheduling Choose general Scheduling Services if you are unsure which facility is correct, need help routing a request, or have care connected to both Brookhaven and Alchemilla.
Tip: If your care involved both facilities, such as an Alchemilla emergency visit followed by Brookhaven review or admission, mention both facilities in your request.

 

Request an appointment through the portal

To request an appointment:

  1. Sign in to your Silent Hill Health patient portal:
  2. Go to Appointments, Schedule a visit, or Request care.
  3. Choose the appointment reason that best matches your need.
  4. Select Brookhaven, Alchemilla, or Silent Hill Health Scheduling Services.
  5. Enter your preferred dates, times, phone number, and contact method.
  6. Add any relevant details, such as recent visit dates, discharge instructions, referral notes, lab needs, imaging needs, or provider instructions.
  7. Upload referral documents, outside records, insurance cards, or discharge paperwork if requested.
  8. Select Submit appointment request.

Why some requests need review

Some appointment requests cannot be confirmed immediately. Scheduling Services may need to review your request before offering an appointment time.

Review may be needed when:

  • The request involves Brookhaven behavioral health intake
  • The appointment requires a referral or insurance authorization
  • A provider must review recent records, labs, imaging, or discharge documents
  • Your records are delayed, restricted, or not linked between facilities
  • The appointment involves a minor, dependent, guardian, or authorized user
  • The requested department has limited availability or facility access restrictions
Tip: Check your portal messages after submitting a request. Scheduling Services may ask for more information before confirming the appointment.

 

Urgent or emergency symptoms

Appointment scheduling is not emergency care. Do not wait for a portal response if you may need immediate medical attention.

Go to the Alchemilla Hospital emergency department or contact local emergency services if you have symptoms that are severe, sudden, worsening, or unsafe to wait.

Important: If your concern involves immediate danger, severe injury, trouble breathing, loss of consciousness, chest pain, signs of stroke, uncontrolled bleeding, or a crisis that cannot safely wait for scheduling review, seek emergency care now.

 

FAQ

I am not sure whether to choose Brookhaven or Alchemilla

Choose Brookhaven for behavioral health care and Alchemilla for medical care, emergency follow-up, outpatient services, labs, imaging, or diagnostics. If your care involved both facilities, choose Silent Hill Health Scheduling Services and include both facility names in your request notes.

My appointment request is still pending

Some requests require provider review, referral review, insurance verification, intake screening, or record review before confirmation. Check your portal messages for requests for more information.

I do not see the appointment type I need

Choose the closest available appointment reason and explain what you need in the request notes. Scheduling Services may reroute the request to the correct department.

The scheduling page looks wrong or keeps refreshing
  • Refresh your browser.
  • Clear your browser cache.
  • Try a different browser or device.
  • Check that you are signed in to the correct patient profile.
  • Wait and try again if the town siren has recently sounded.
  • Avoid using public terminals located in restricted offices, closed wards, or archive areas.
