Schedule a Brookhaven behavioral health appointment

Use this article to request a behavioral health appointment with Brookhaven Hospital. Brookhaven scheduling may include intake review, psychiatric evaluation, therapy or counseling follow-up, medication management, post-discharge follow-up, or crisis follow-up after care at Alchemilla Hospital.

Some Brookhaven appointments cannot be scheduled immediately through the portal. Requests may require provider review, intake screening, privacy review, records review, or department approval before an appointment time is confirmed.

Note: Brookhaven appointment availability may change during intake review periods, provider coverage changes, restricted ward access, records-room service interruptions, lake fog advisories, or townwide siren events.

 

Brookhaven appointments you can request

Choose the appointment reason that best matches the care you need. Scheduling Services may adjust the request type after review.

Behavioral health intake For new patients, returning patients after a long gap in care, or patients who need their care needs reviewed before scheduling.
Psychiatric evaluation For diagnostic review, medication planning, treatment recommendations, or provider-requested psychiatric assessment.
Therapy or counseling follow-up For ongoing therapy, counseling, treatment-plan follow-up, or provider-recommended behavioral health support.
Medication management For medication review, refill-related follow-up, medication changes, side-effect concerns, or monitoring appointments.
Post-discharge follow-up For follow-up after Brookhaven inpatient care, discharge planning, transfer review, or care coordination.
Crisis follow-up after Alchemilla care For behavioral health follow-up after an Alchemilla emergency visit, safety review, medical clearance, or referral to Brookhaven.

  • Your preferred phone number, email address, or portal message preference
  • The reason for the appointment
  • Any recent Brookhaven visit, discharge, or treatment-plan information
  • Any recent Alchemilla emergency visit connected to the request
  • Current medications, if requesting medication management
  • Referral information, if another provider told you to schedule with Brookhaven
  • Insurance information, if coverage review may be needed
  • Guardian, caregiver, or authorized-access information, if someone else helps manage your care

Request a Brookhaven appointment through the portal

To request a Brookhaven behavioral health appointment:

  1. Sign in to the Brookhaven patient portal:

  3. Select the appointment reason that best matches your need.
  4. Choose Brookhaven Hospital as the facility.
  5. Enter your preferred dates, times, callback number, and contact method.
  6. Add details about recent care, discharge instructions, medication needs, referral notes, or Alchemilla emergency follow-up.
  7. Upload referral documents, outside records, discharge paperwork, or authorization forms if requested.
  8. Review the request summary and select Submit Brookhaven appointment request.

Provider review and intake screening

Brookhaven appointment requests may be reviewed before scheduling. This helps confirm that the appointment type, provider, timing, and care setting are appropriate for the request.

Review or screening may be needed when:

  • You are requesting a first-time behavioral health intake
  • You are returning to care after a long gap
  • The request involves psychiatric evaluation or medication management
  • You were recently discharged from Brookhaven
  • You were referred after an Alchemilla emergency visit
  • A provider needs to review recent records before scheduling
  • The appointment involves a minor, dependent, guardian, or authorized user
  • The request includes safety planning, crisis follow-up, or care coordination needs

Brookhaven appointment availability may be limited by provider schedules, intake capacity, care level, and department review. If an appointment is not available right away, you may be offered a waitlist option.

Depending on the request type, you may be able to:

  • Request a callback if a provider opening becomes available
  • Ask whether another Brookhaven provider can review the request
  • Request follow-up after an Alchemilla emergency visit

Urgent or crisis concerns

Brookhaven scheduling requests are reviewed during normal scheduling workflows and are not emergency care. Do not wait for a portal response if there is immediate danger or if symptoms cannot safely wait.

Use urgent or emergency support if you or someone else may be at immediate risk, cannot stay safe, has worsening symptoms, or needs immediate medical or crisis evaluation.

Important: For immediate medical or safety concerns, go to the Alchemilla Hospital emergency department or contact local emergency services. Portal scheduling should not be used for urgent crisis response.

I was referred after an Alchemilla emergency visit

Include the Alchemilla emergency visit date, discharge instructions, and referral details in your Brookhaven request. Scheduling may need to review or link the Alchemilla record before confirming behavioral health follow-up.

I do not see the Brookhaven appointment type I need

Choose the closest available appointment reason and explain what you need in the request notes. Brookhaven Scheduling may update the appointment type after reviewing your request.

I cannot schedule because my records are not linked

If Brookhaven and Alchemilla records are not connected, Scheduling Services may need the profiles reviewed before confirming care. Submit a profile-linking or duplicate-account request if records appear split between facilities.

The scheduling page looks wrong or keeps refreshing
  • Refresh your browser.
  • Clear your browser cache.
  • Try a different browser or device.
  • Check that you are signed in to the correct patient profile.
  • Wait and try again if the town siren has recently sounded.
  • Avoid using public terminals located in restricted offices, closed wards, or archive areas.
