Update your appointment preferences

Bring the information and documents needed to confirm your identity, verify coverage, review your care needs, and help staff prepare for your visit. This checklist applies to most Silent Hill Health appointments, including Brookhaven Hospital visits, Alchemilla Hospital outpatient appointments, emergency follow-up, lab work, imaging, diagnostic testing, specialist referrals, and procedure follow-up.

Some appointments may require additional items based on the facility, department, provider instructions, or reason for the visit. Check your portal messages before arriving.

Note: Arrival instructions may change during department closures, lake fog advisories, road access restrictions near Toluca Lake, records-room service interruptions, or townwide siren events.

Items to bring to every appointment

For most visits, bring:

  • Photo ID or other accepted identification
  • Insurance card, if you have insurance
  • Current medication list, including dose and frequency
  • Allergy information
  • Preferred pharmacy information
  • Your Silent Hill Health Patient ID, if available
  • Appointment confirmation or portal message
  • Any forms, instructions, or documents requested by the facility

Medical records, orders, and instructions

Bring any records or instructions that may help your care team understand why you are being seen.

Referral paperwork Bring referral forms, referral numbers, provider instructions, or outside specialist recommendations if your appointment was referred.
Discharge instructions Bring Alchemilla emergency department instructions, inpatient discharge papers, procedure instructions, or Brookhaven discharge planning documents.
Lab or imaging orders Bring lab orders, X-ray orders, imaging instructions, diagnostic testing instructions, or outside provider orders if requested.
Prior records Bring outside records, prior visit summaries, test results, medication history, or records from another provider if they are not already in your portal.

Insurance, billing, and payment information

Bring billing and coverage information so registration can verify your account.

  • Insurance card
  • Secondary insurance card, if applicable
  • Referral or authorization number, if your insurance requires one
  • Billing account or guarantor number, if the visit is tied to an existing billing account
  • Payment method for copay, deposit, or estimated patient responsibility
  • Updated billing address, if your address recently changed
  • Workers’ compensation, school, employer, or legal claim information, if applicable

Guardian, caregiver, or authorized-access documents

If someone else helps manage your care, staff may need to confirm their role before discussing appointment details, records, billing, or treatment information.

Depending on the visit, bring:

  • Guardian documentation
  • Custody or dependent-care documents
  • Healthcare proxy or authorized representative documentation
  • Medical release form, if records need to be shared
  • Caregiver contact information
  • Support person name and relationship to the patient
  • Documents requested by Brookhaven, Alchemilla, or Privacy Services
Important: Bringing a support person does not automatically give them access to your medical records. Authorized access must be approved through the correct records or portal-access process.

Accessibility, mobility, and service animal needs

If you need accessibility or mobility support, add a note to your appointment request or contact Scheduling Services before your visit.

You may want to bring or request:

  • Mobility device, such as a cane, walker, wheelchair, or brace
  • Assistive communication device
  • Hearing, vision, or communication support instructions
  • Quiet waiting area request, if available
  • Service animal note, if needed
  • Transportation or drop-off instructions
  • Support person details, if someone will help you during the visit
Note: Some areas may have limited access because of clinical safety, privacy review, restricted corridors, emergency department activity, or closed ward controls.

Brookhaven and Alchemilla visit notes

The items you need may depend on the facility and appointment type.

Brookhaven Hospital Bring medication lists, treatment-plan notes, discharge documents, caregiver or authorized-access documents, and any intake forms requested by Brookhaven Scheduling.
Alchemilla Hospital Bring ID, insurance, discharge instructions, referral paperwork, lab or imaging orders, medication lists, and any preparation instructions for outpatient, emergency follow-up, diagnostic, or procedure-related visits.
Shared-care appointments If your care involved both Brookhaven and Alchemilla, bring visit dates, discharge instructions, referral notes, and any records that may not yet be linked in your portal.

FAQ

I forgot my photo ID

Contact the facility before arriving if possible. Registration may ask for another accepted form of identification or may need staff review before check-in can continue.

I do not have my insurance card

Bring any insurance information you have, including member ID, group number, or a digital card. Patient Financial Services may still need to verify coverage before billing is finalized.

My outside records are not ready

Attend the appointment unless the facility tells you otherwise. Let registration or your care team know that outside records are still pending. Some visits can proceed while records are requested or reviewed later.

I need to bring a caregiver or support person

Check your appointment instructions and facility visitor rules. If the support person needs to receive appointment or record information, make sure authorized access is already in place.

I am not sure where to go

Check your appointment confirmation for the facility, department, entrance, and arrival instructions. If the instructions are unclear, contact Scheduling Services before traveling through restricted roads, fog advisory areas, or facility back corridors.
